Test Descriptions

Most patient workups performed in the HLA Laboratory involve more than one test.  Please refer to the test profile description for sample requirements and test to be ordered.  For further assistance, please contact the HLA laboratory at (509) 232-4490.

HLA Class I (ABC) Typing 
CPT Code (suggested):  86813 

HLA Class I (ABC) typings are performed by low-resolution sequence-specific primer (SSP) amplification.  This test can be performed using anticoagulated peripheral blood, lymph nodes or spleen. 

HLA Class II (DRDQ) Typing 
CPT Code (suggested):  86817 

HLA Class II (DRDQ) typings are performed by low-resolution sequence-specific primer (SSP) amplification.  This test can be performed using anticoagulated peripheral blood, lymph nodes or spleen. 

HLA B27 Typing 
CPT Code (suggested):  86812 

HLA B27 typings are performed by complement dependent cytotoxicity using lymphocytes isolated from peripheral blood. 

Flow Cytometric Lymphocyte Crossmatch 
CPT Code (suggested):  86805 

Patient serum is crossmatched with donor lymphocytes to detect donor specific antibodies in the patient serum.  Patient serum is also crossmatched against the patient lymphocytes to detect autoantibodies.  Antibody binding is visualized through fluorescent-labeled secondary antibodies on the flow cytometer.

Cytotoxic Lymphocyte Crossmatch

Patient serum is crossmatched with donor lymphocytes to detect donor specific antibodies in the patient serum.  T cell crossmatches are performed by antigloublin augmented complement dependent cytotoxicity and B cell crossmatches are performed by AMOS modified complement dependent cytotoxicity.   

Autologous Lymphocyte Crossmatch 
CPT Code (suggested):  86805 

Patient serum is crossmatched with the patient’s own lymphocytes to detect autoantibodies.  T cell crossmatches are performed by antigloublin augmented complement dependent cytotoxicity and B cell crossmatches are performed by AMOS modified complement dependent cytotoxicity.   

T-cell Antibody Screen 
CPT Code (suggested):  86807 

Patient serum is screened against a panel of lymphocytes on a commercial tray by antiglobulin enhanced complement dependent cytotoxicity.  Results are expressed as the percent reactive antibody (PRA), which is the number of lymphocytes that reacted positively with the patient serum divided by the total number of lymphocytes tested.  In some cases antibody specificities can be defined by this test.  

FlowPRA I/II Screen 
CPT Code (suggested):  86807 

Patient serum is screened against a panel of beads coated with purified class I and class II HLA antigens and acquired by flow cytometry.  Results are expressed as the percent reactive antibody (PRA), which is the percentage of beads that reacted positively with the patient serum.  Antibody specificity cannot be defined by this test. 

FlowPRA Antibody Identification 
CPT Code (suggested):  86807 

Patient serum is screened against beads coated with only one or two purified HLA antigens and acquired by flow cytometry.  This method is used for antibody identification and does not generate a PRA.

HPA1 Platelet Genotyping 
HPA1 platelet genotyping is performed by sequence specific primer (SSP) amplification.  Patients will be genotyped as either HPA1a homozygous, HPA1a/HPA1b heterozygous, or HPA1b homozygous. 

Deceased Donor Preliminary Crossmatches 
CPT Code (suggested):  86805

Donor lymphocytes are crossmatched against ABO compatible heart candidates.  T cell crossmatches are performed by antigloublin augmented complement dependent cytotoxicity and B cell crossmatches are performed by AMOS modified complement dependent cytotoxicity. 

HLA Matched Platelet Donor Search 
CPT Code (suggested):  86023

For patients requiring HLA compatible platelets, an HLA donor search is performed in order to generate a list of compatible donors to be recruited. 

Call Back Fee 
CPT Code (suggested):  99058

A call back fee is charged whenever stat testing must be done outside of normal workday hours.
